Parent/Guardian Waiver, Assumption of Risk & Release
As the parent or legal guardian of the participating minor athlete (“Athlete”), I understand that participation in PacWest Endurance’s Youth Development, Junior Elite, Development Team, Adaptive Athlete, camps, clinics, performance testing, strength and conditioning, swimming, cycling, running, races, virtual coaching, and all related activities (“Activities”) involves inherent risks that may result in serious injury, permanent disability, or death.
I certify that my child is physically able to participate and that I have disclosed any known medical conditions, allergies, medications, or limitations that may affect participation.
I acknowledge that PacWest coaches cannot eliminate all risks associated with endurance sports and that accidents may occur despite reasonable supervision.
I voluntarily assume all risks on behalf of my child.
To the fullest extent permitted by law, I release and agree to hold harmless Pacific West Endurance LLC (PacWest Endurance), its owners, coaches, employees, contractors, volunteers, sponsors, partners, facility providers, and affiliates from any claims arising from my child’s participation, including claims arising from ordinary negligence.
Parent & Athlete Responsibilities
The parent/guardian agrees to:
- Ensure the athlete arrives prepared with appropriate equipment, hydration, nutrition, and clothing.
- Promptly communicate injuries, illnesses, medications, or health concerns.
- Ensure the athlete follows coach instructions and demonstrates respectful behavior.
- Pick up the athlete promptly following scheduled activities unless otherwise arranged.
The athlete agrees to:
- Treat coaches, teammates, volunteers, officials, and facilities with respect.
- Follow all safety instructions.
- Wear required safety equipment.
- Demonstrate good sportsmanship and positive behavior.
PacWest reserves the right to remove an athlete from any activity when safety or conduct becomes a concern.
Medical Authorization
In the event of an illness or emergency and the parent or emergency contact cannot be reached, I authorize PacWest personnel to obtain emergency medical treatment for my child when deemed necessary.
I understand that I am responsible for all medical expenses incurred.
Training & Program Policies
I understand that:
- Training plans may be modified based upon athlete development, weather, facility availability, safety concerns, or coaching discretion.
- Participation in competitions is based upon athlete readiness, safety, and coach recommendation.
- Attendance does not guarantee selection for specific teams, travel opportunities, championships, or elite events.
Membership, Billing & Refunds
- Memberships automatically renew according to the selected billing cycle unless cancelled in writing at least thirty (30) days before renewal.
- Coaching fees, memberships, camps, clinics, and event registrations are non-refundable unless otherwise stated.
- Credit card charges may appear as PacWest*Endurance.
Photography & Media
I grant permission for PacWest Endurance to photograph or record my child during Activities for educational, promotional, marketing, website, social media, and recruitment purposes unless I submit a written request opting out prior to participation.
Communication
I understand that PacWest communicates with parents and athletes through email, text messaging, training platforms, and other digital communication tools. I consent to receiving these communications regarding programming, scheduling, safety updates, and team information.
