Youth & Junior Membership Parent/Guardian Waiver, Release from Liability & Policies

Parent/Guardian Acknowledgement

I am the parent or legal guardian of the youth athlete identified during registration (“Athlete”).

I understand that participation in PacWest Endurance activities, including swimming, cycling, running, strength training, camps, clinics, group workouts, races, and community events involves inherent risks.

I understand that endurance sports may involve:

Falls and collisions.
Road, trail, and environmental hazards.
Physical exertion.
Weather-related risks.
Equipment-related risks.
Serious injury, disability, or death.

I voluntarily accept these risks on behalf of myself and my child.

Release of Liability

To the fullest extent permitted by law, I release and hold harmless Pacific West Endurance LLC (PacWest Endurance), its owners, coaches, employees, volunteers, partners, sponsors, affiliates, and facilities from any claims arising from my child’s participation in PacWest activities, including claims arising from ordinary negligence.

Parent/Guardian Responsibilities

I agree to:

Provide accurate medical and emergency contact information.
Inform PacWest of injuries, medical conditions, allergies, or limitations.
Ensure my athlete arrives prepared with proper equipment, hydration, and clothing.
Ensure transportation arrangements are made before and after activities.
Support positive behavior and sportsmanship.

Athlete Expectations

The athlete agrees to:

Respect coaches, teammates, volunteers, and facilities.
Follow safety instructions.
Treat others with respect.
Represent PacWest Endurance positively.

PacWest reserves the right to remove an athlete from activities due to unsafe behavior, misconduct, or failure to follow team expectations.

Medical Authorization

In the event of an emergency where a parent/guardian cannot immediately be reached, I authorize PacWest personnel to seek appropriate emergency medical treatment for my child.

I understand that I am responsible for all related medical expenses.

Youth Membership Policies

Youth Membership benefits may include:

PacWest community affiliation.
Access to youth-oriented events and activities.
Discounts and member benefits.
Training resources and educational opportunities.

Membership does not guarantee placement on a competitive team, race selection, coaching services, or advancement within PacWest programs.

Membership Renewal & Cancellation

Annual memberships automatically renew unless cancelled before renewal.
Fees are non-refundable once processed.
Credit card charges may appear as PacWest*Endurance.

Photography & Media

I grant PacWest Endurance permission to use photographs or video taken during PacWest activities for promotional, educational, website, and social media purposes unless I submit a written opt-out request.
